Click on the link "Evaluation form" for the desired date.

Click on the save or download icon (generally located either at the top or bottom right in your new browser window).

Right click on the download PDF file and select “Open with system viewer”. This will open the form in Acrobat Reader (or a similar program that is on your computer).

Fill out the form and click submit, which will send it via email to Sally Perry.
